Laser Therapy

Cold Laser Therapy

Also known as low-level laser therapy (LLLT), cold laser therapy has been shown to help decrease pain and musculoskeletal discomfort by improving blood flow and oxygenation and decreasing inflammation. The treatment utilizes specific wavelengths of light to non-invasively penetrate tissue without pain. It has proven successful in treating muscle pain, arthritis, fibromyalgia, joint pain (especially of the shoulder and neck), carpal tunnel syndrome and other conditions associated with pain, inflammation and nerve irritation.

We use the LaserQuant device which works in two areas on the body simultaneously or on two patients at the same time. It combines low level pulse laser radiation with other radiations and clinical therapeutic modalities pulsating infrared radiation, visible red light and static magnetic fields, providing their synergic therapeutic effects.

Incorporating all of these radiations into one apparatus achieves rapid and efficient clinical effects within a relatively short time. LaserQuant incorporates into one probe four distinct therapeutics:

Muscle and ligament strains, pain is usually caused by ischemic injury to tissue due toreduced microcirculation and impaired supply of blood to the cells. LaserQuant incorporates into one probe all of the known radiations that are capable of transforming living cells from an unstable state of illness into a stable, healthy state. Applying the LaserQuant probe to areas of pain results in improved microcirculation, improved supply of oxygen, and removal of toxic waste products. As a result, the tissue is cured and pain disappears.

Listed below are sample case studes of some of the treatments that LaserQuant has been used for:

Epicondylitis

42-year-old female with right lateral epicondylitis, pain extending into the extensor area of the forearm (out-cropping muscles). She was instructed in the use of a tennis elbow strap, massage of the extensor muscles and stretching of the muscle bellies. She was also asked to use a topical anti-inflammatory ointment. After presenting with extreme pain, she reported increasing benefit after the first 4 treatments with Low Level Laser Therapy, and was 90% -95% pain-free after 7 treatments. She was encouraged to continue other supportive measures.

Cramps

73-year-old female presented with severe pain in the left calf following an episode of prolonged cramps while in bed. She was seen to be extremely tender over the medial head of the Gastrocnemius muscle, and responded well to 4 treatments with low level laser.

63-year-old insulin-dependent diabetic with severe degenerative disease of the right knee, waiting for knee replacement. Awaiting hospital admission, she received daily and alternate day treatment over 7-10 days on 2 occasions to buy time until surgery was carried out. Post-operatively, she was using an exercise bicycle during her rehabilitation, and developed muscle strain of the upper anterior compartment muscles. Also post-operatively, she developed a pressure ulcer on her other heel. Her anterior compartment strain improved daily with 5 treatments, and the moderately deep pressure ulcer which had been closing slowly showed rapid improvement from 1.5cms to 2mm in 7 days and was closed by the 12th day.

25-year-old male who sustained a mid-shaft fracture of the humerus on 20th July in Darwin while pitching a baseball. He was treated conservatively in Darwin. After 6 weeks he came to Melbourne and complained of pain around the upper arm and in the cubital fossa area. After investigating him for osteoporosis (neg.) he was started on low-level laser therapy and given exercises to increase strength around the elbow and shoulder. He showed good recovery after 14 days' treatment as regards, pain but returned 1 week later, requesting further treatment from which he derived significant improvement.

Foot and knee Pain

32-year-old male knocked right knee over upper margin of patella. He complained of pain and weakness on extension of knee. After 3 treatments of low level laser therapy, pain and weakness had subsided.

57-year-old female had a large tin of canned fruit dropped onto dorsum of right foot. Patient on Warfarin. She developed pain and swelling (haematoma) over the dorsum of her foot. After 4 treatments, she was able to walk comfortably with little pain on extension of ankle joint and toes.

Achilles Tendonitis

Age 34: The acute injury occurred while playing basketball, which he played only occasionally.  The Achilles tendon was swollen and acutely painful at palpation.  The patient was walking with moderate pain.

Treatment consisted of three 10-minute sessions of LaserQuant (10 minutes @ 50Hz) for one week, combined with Non Steroidal Anti-Inflammatory Drugs (NSAID).

At the end of the treatment, the patient was able to walk freely; the Achilles tendon was not painful at palpation.

Age 78: This patient had bilateral Achilles tendonitis and bilateral plantar fasciitis for several weeks, with increasing intensity.  The pain had not been relieved with paracetamol.  The injuries markedly annoyed this patient since he was responsible for the maintenance of his local bowling club and was an ardent player.

Treatment consisted of 6 sessions of LaserQuant, combined with NSAID.  Each LaserQuant session consisted of 10 minutes at 50Hz for each of the Achilles tendons and each of the plantar fasciitis. The LaserQuant sessions were distributed over a two-week period.

At the end of the treatment period, this gentleman was pain-free, and fully enjoyed his bowling again.

Shoulder bursitis

Age 28: The injury occurred at work; while digging and shoveling heavy wet earthy clay, the shovel got stuck under a rock, and the patient exerted undue force to try to lift the rock, which did not move an inch.  The patient is very muscular and exercises regularly (weight-lifting).  The biceps tendon was painful and the was limited abduction at 60 degrees and very limited internal rotation and forward flexion.  An ultrasound showed biceps tendon inflammation and partial tear.

The treatment consisted of 4 sessions of LaserQuant (10 minutes @ 50Hz) with a prescription for NSAID and ice packs.  He was assigned to light duties with a restriction on lifting no more than two kilos.  The four sessions of LaserQuant were given over a 10-day period.

At the end of the treatment, the patient was pain-free and there was no limitation of movement, and palpation was pain-free.

Lateral Epicondylitis

Age 43: This electrician had been suffering from lateral epicondylitis for several months, with increasing intensity, not only while using his forearm but also while at rest and even at night.  Courses of NSAID taken intermittently had provided in substantial relief.

The treatment consisted of 6 sessions of LaserQuant (10 minutes at 50 Hz) over a period of three weeks, combined with 3 myotherapy treatments and a course of NSAID.

After the three weeks of treatment, the patient was pain-free, both while using his arm and while at rest, and at palpation of the lateral epicondyle.

Hip bursitis

Age 72: This retired woman was suffering from left hip pain that reduced her ability to go for long walks and also prevented her from sleeping on her left side.  Getting up from a chair or her sofa was painful.  Palpation of the great trochanter was painful, and a left hip ultrasound confirmed bursitis.

The treatment consisted of eight treatments of LaserQuant (5 minutes variable frequency and 5 minutes at 50 Hz) over a three-week period.

At the end of the treatment, the patient was pain-free and could enjoy her walks again.  There was no pain at palpation and no limited range of movement of her left hip.

Coccyx pain

Age 84 -  This patient had sustained a fractured coccyx several years earlier and remained very sore.  She could not sit straight for long.  Traveling by car was limited to a few kilometers because of the pain. Analgesics, NSAIDs, and acupuncture had had no effect.

The treatment consisted of 12 sessions of LaserQuant (10 minutes at 5 Hz) over a four-week period.

After six treatments, the patient was able to sit straight albeit with bearable discomfort.  After 12 treatments, she was pain-free and could walk and travel by car without restriction.

Superficial Thrombophlebitis

Age 54: the patient had a superficial thrombophlebitis of the leg for several days. Treatment consisted of two LaserQuant sessions of 10 minutes at 1000Hz on two consecutive days. After the second treatment, the inflammation had disappeared and the leg was not sore.

Shingles

Age 24: This patient had developed herpes zoster on a thoracic dermatoma.  He presented 6 days after the occurrence of symptoms.

The treatment consisted of 4 sessions of LaserQuant (10 minutes at 1000Hz) over a ten-day period, with a prescription of tramadol and paracetamol 500mg and 30 mg of codeine.

After the treatments, the pain had markedly diminished, and an NSAID was sufficient to control it.  The skin lesion had almost disappeared.

Osteoarthritis

A patient suffering from osteoarthritis with severe pains in both knees, and above and under the knees.

On VAS scale, levels of pain: 8-9.

After 3 sessions of treatment with the laser device, level of pain was reduced to VAS 6; following an additional 7 sessions, reached VAS 3. Altogether, after 12 treatments a marked improvement in pain was noted.

KAPLAN MEDICAL CENTER

Tendopains

A patient suffering from severe pains in dorsal part of foot. The affected area was swollen and very sensitive. The case was diagnosed as Tendopainc.

Following 2 treatments with the laser device, the area was less swollen, and following an additional 7 treatments, complete healing was noted, with completely free movement of the foot.

Non-bacterial inflammation

A patient suffering from severe pains in the penis, due to non-bacterial inflammation. All conventional treatments were ineffective. Following 3 treatments with the laser device, level of pain was reduced, and after an additional 6 treatments, pains practically disappeared.

Planter Fasciitis

A patient suffering from severe pain in the left foot and unable to stand on it.  Upon X-ray, Planter Fasciitis due to a spur was diagnosed. On the VAS scale, pains were at the level of 6-7. Following 8 treatments with the laser device, pains were reduced to level 2, and the patient started to walk freely.

Muscle Pain

43-year-old female with incomplete tear of the inner thigh muscles sustained after a cycling injury. A reduction in pain was noticeable after the first treatment, and a dramatic decrease in swelling after a series of treatments.

Back Pain

52-year-old male with pure low back pain, 3 months post-discectomy. The treatment was focused on the Para vertebral muscles in the lumbar spine.  A decrease in pain was obtained and after the third treatment he was able to return to physical therapy (which he had stopped due to the severity of the back pain).

Chronic Shoulder Tenosynovitis

42-year-old male with chronic shoulder tenosynovitis, an occupationaly induced disorder. In this case, he works over 7 hours a day at a computer work station. A reduction in pain and an improvement in his active shoulder movements were obtained after a series of 5 treatments. Further improvement was attained after additional LLLT and physiotherapy.

Lateral Epicondylitis

1. 35-year-old female with lateral epicondylitis (Tennis Elbow). She had not responded to standard treatments with NSAID and ultrasound, but did have a significant reduction in pain after a series of 6 LLLT treatments.

2. 42-year-old female with right lateral epicondylitis, pain extending into the extensor area of the forearm (out-cropping muscles). She was instructed in the use of a Tennis Elbow strap, massage of the extensor muscles, and stretching of the muscle bellies. She also was asked to use a topical anti-inflammatory ointment. Bursitis

73-year-old female with a painful left shoulder. Ultrasound of shoulder demonstrated bursitis and fluid around the biceps tendon. Pain worse at night. Limited range of movement. LaserQuant treatment was commenced daily for 6 days, with notable improvement in level of pain. She was virtually pain-free during the day, with diminishing night pain and increasing range of movement of the shoulder joint.

Achilles Tendonitis

Wounds

79-year-old female, personal history of dementia and osteoporosis. A fall from her bed on 06 January 2004 resulted in a large skin tear. Due to her dementia, she pulled the dressings off and painted lipstick around the wound. With the introduction of the LLLT (programs 2 & 4) on 30 April 2004, and wound protocol of Intrasite gel, the wound had healed by the 20 May 2004.

PHX of IDDM

79-year-old female, with PHX of IDDM, blindness, amputation of (L) toes (11 Feb 2000) and (R) middle toe (June 1999). An ulcer developed on her left foot in May 2001 and she was sent to hospital for investigation for gangrene.  A sinus developed in the original wound on 20 Jul 2001, which we continued to pack with Kaltostat; and eventually the wound healed. In Feb 2002, the wound returned. A wound specialist was consulted, and a full felt pad was recommended to reduce the pressure.  On 21 Oct 2003, pus was expelled and sinus noted. The patient was again hospitalized and her diagnosis of cellulitis was treated with antibiotics. The caregiver continued to dress the wound daily, beginning with October 2003, with no signs of improvement.

On 30 Apr 2004, the wound was measured at 2.8cm x 2.8cm. LaserQuant treatment was commenced using programs 2 & 4. By 17 May 2004, the size had reduced to 1.8cm x 0.8cm. On 06 Jun 2004, the patient was referred to the podiatrist, who applied felt to reduce pressure. By 16 Jun 2004, without changing the dressing protocol, the wound had decreased and measured at 0.7cm x 0.3cm. Wound was re-measured on 29 Jun 2004, at 0.7cm x 0.1cm. The attending physician was very impressed with the outcome, particularly given the patient’s history.

Multiple Sclerosis

94-year-old female, with a past history of multiple sclerosis, Arthritis and repetitious leg venous ulcers. This lady complained of severe pain in her left and right knees, and a raised rash was noted on her left leg.

LaserQuant programs 1 (5Hz) & 3 (1000Hz) treatments were introduced on 10 May 2004, and the rash was resolved within 3 days. On 18 Jun 2004, the resident complained of pain in her (R) shoulder, and on examination by her general practitioner, she was diagnosed with a ruptured tendon. Laser treatment was recommended. Treatment was continued on all three areas using programs 1 & 3, with a positive result. Pain was reduced and she now has the ability to ambulate more freely. Long term MS Contin was prescribed in conjunction with programs 1 & 3.
